Lodge Donation Report for Sending CHEMPI Donations to the CHEA office

This form is used by the Lodge Secretary to be sent to the CHEA Office, 5450 E. Lamona Ave., Fresno, California, 93727, with donations. The purpose of this form is to ensure that the member and the Lodge are credited accurately for their donation.
District Piggy Bank Chairman Monthly Report Form

To be submitted to the Association Chairman and Vice Chairman by the 10th of each month.
Lodge Piggy Bank Chariman Monthly Report Form

This report should be sent to your District Chairman no later than the 8th of the month to report the figures for the previous month.
